Job summary

Pacific Health Group seeks a Member Engagement Specialist to drive outbound outreach, educate eligible individuals about ECM and PHG services, and facilitate smooth enrollments. You will coordinate with health plans, providers, and community partners to ensure timely access to services.

The role requires empathy, persistence, and strong organizational skills to support underserved populations while maintaining HIPAA-compliant documentation and accurate record-keeping.

Qualifications

• High school diploma or equivalent required.
• Associate's or Bachelor's in social work, public health, human services, healthcare administration, or related field preferred.
• Minimum 1-2 years of experience in customer service, healthcare, or a related field.
• Excellent verbal communication and interpersonal skills.
• Comfortable spending the majority of the workday conducting outbound member calls.
• Strong organizational and time-management abilities; reliable transportation and driver's license for events.

Responsibilities

• Outreach to prospective members via outbound calls and referrals.
• Educate members on Enhanced Care Management (ECM) and PHG services.
• Build rapport and establish trust with members through compassionate communication.
• Complete enrollments and obtain required consents and documentation.
• Coordinate referrals and warm handoffs to support transitions into ECM services.
• Track referral status and follow up with all prospective members.
