Job summary

Pacific Health Group is seeking a Member Engagement Specialist to play a critical role in outreach, enrollment, and care coordination. You will educate eligible individuals about ECM and PHG services, help with Medi-Cal applications, and ensure smooth transitions into ongoing care management with PHG teams.

The position involves outbound calling, relationship-building with referral partners, and community outreach to promote awareness and access to PHG services.

Qualifications

• High school diploma or equivalent is required.
• Associate’s or Bachelor's degree preferred.
• 1–2 years experience in customer service, healthcare, or related field.
• Excellent verbal communication and interpersonal skills.
• Experience with outbound calls and care coordination is a plus.

Responsibilities

• Conduct outbound outreach to educate eligible individuals about PHG programs.
• Assist with Medi-Cal and BenefitsCal applications when appropriate.
• Enroll members and obtain required consents and documentation.
• Coordinate with Lead Care Managers for transitions into ECM services.
• Represent Pacific Health Group at community events to promote services.
