Job summary

Pacific Health Group in Las Vegas, NV is seeking a Member Engagement Specialist to serve as the first contact for prospective members. You will conduct outbound outreach, educate on ECM and PHG services, and help with enrollment and enrollment documentation.

The role focuses on building trust with community partners, coordinating with Lead Care Managers, and supporting Medi-Cal applications while ensuring timely access to services.

Qualifications

• First point of contact for prospective members; educate on ECM and PHG services.
• Assist with enrollments and required consents/documentation.
• Coordinate referrals and warm handoffs to Lead Care Managers.
• Assist with Medi-Cal/BenefitsCal applications when appropriate.

Responsibilities

• Conduct outbound telephone outreach to prospective members referred by plans, providers, hospitals, and community organizations.
• Educate about Enhanced Care Management (ECM) and PHG services.
• Build rapport and trust; complete enrollments and obtain consents.
• Coordinate with care teams for smooth transitions to ECM services.
• Track referrals from receipt to enrollment and follow up.
