Qualifications

• High school diploma or equivalent required.
• Associate's or Bachelor's degree in related fields preferred.
• Minimum 1-2 years in customer service or healthcare settings preferred.

Responsabilités

• Conduct outbound outreach to educate eligible individuals about PHG programs.
• Assist with Medi-Cal and BenefitsCal applications and enrollments.
• Coordinate with care managers for smooth transitions into ECM and CS services.
• Represent PHG at community events to promote services.

Formation
