Premise Health is seeking a Full Time Referral Coordinator in Orlando, FL to support client employees and dependents in our Health Center vicinity. You will work with the care team to identify health goals and access needed services, while coordinating appointments and transportation as required.

Responsibilities include documenting in Epic, attending meetings with partners, and ensuring a shared care plan across the team for optimal health outcomes.

Qualifications

• High School diploma or GED required.
• Prior experience accessing and navigating community resources preferred.

Responsabilités

• Identify and prioritize members' health care goals with the care team.
• Facilitate access to medical and social services for members and dependents.
• Schedule appointments and arrange transportation to ensure access to services.
• Document encounters and follow-up plans in Epic.
• Participate in case conferences to align care plans.
