Responsibilities

• Perform medication history interviews with patients.
• Verify home medication lists with pharmacies or offices.
• Document all identified medications in the electronic medical record with accuracy.
• Discuss medication history issues with clinical pharmacist staff to improve care transitions.
• Provide training and development for technicians and students.
• Participate in quality improvement activities and committee work.
• Perform related duties as needed.
