Job summary

Duke Health is seeking a Pharmacy Tech I/II (Part-Time) to perform medication history interviews for patients at Duke University Hospital in Durham, NC. This role works with the Transitions of Care Manager within Clinical and Patient Care Services to ensure accurate medication histories and high-quality transitions of care.

The position requires high school diploma or equivalent, completion of a technician program, NC Board registration within 180 days, and PTCB/ExCPT certification.

Qualifications

• Knowledge of medication names (brand/generic) and basic math.
• Attention to detail and persistence in work ethic.
• 5 years (Community/Retail setting preferred).
• Experience performing medication histories/reconciliation is preferred.
• Able to work independently and prioritize in a high-volume environment.
• Strong communication and interpersonal skills with patients and co-workers.

Responsibilities

• Perform patient interviews to obtain a complete home medication list.
• Verify medication history by contacting pharmacies or physicians.
• Document all identified medications in the EMR accurately.
• Discuss medication history issues with clinical pharmacist staff.
• Provide medication history-related training for technicians and students.
• Participate in quality improvement activities focused on care transitions.
• Attend committee or team meetings and contribute to projects.
