This full-time hybrid role based in Phoenix involves coordinating care between patients, providers, and pharmacy teams, performing reconciliation, and driving continuous quality improvement.

Qualifications

• Minimum of 2 years of experience in healthcare or population health.
• Experience with medication reconciliation and patient education.
• Experience in home healthcare or related clinical setting preferred.

Responsibilities

• Support the clinical team to improve patient outcomes through medication management and adherence initiatives.
• Assist with population health programs and quality improvement projects.
• Coordinate care between patients, providers, and pharmacy teams; perform medication reconciliation.
• Travel to healthcare facilities or patient homes as part of outreach programs.
