Job summary

ChenMed in Miami, FL is seeking a Medication Adherence Technician to engage patients and collaborate with providers, pharmacists, and the pharmacy team to resolve adherence barriers and ensure safe, coordinated care.

You will make outbound calls, remove barriers to prescriptions, coordinate prescription processing, and support adherence workflows while upholding exceptional patient experiences.

Qualifications

• High School Diploma or equivalent; related experience may substitute.
• Certification as Pharmacy Technician or Medication Assistant with at least 6 months of related work experience OR 2 years in a medication dispensing/pharmacy setting or call center.

Responsibilities

• Makes outbound calls to patients and healthcare professionals regarding patient medications, adherence, and barrier removal.
• Removes barriers to ensure prescription regimens are provided to patients; involves analysis, research and problem-solving.
• Interacts with healthcare professionals at all levels; assists with refills, synchronization, or 90-day supply requests.
• Captures and supports medication adherence prescription requests/needs.
• Coordinates prescriptions sent to the pharmacy, processed, adjudicated, and prepared for dispensing.
• Follows established medication adherence workflows for enrollment and ongoing delivery.
• Ensures tasks are accurate and reviewed before dispensing.
• Meets daily outreach targets for medication adherence measures.
• Provides feedback and/or solutions to improve patient experience.
• Assists with training programs for clinical personnel involved in Part D HEDIS measures.
• Performs other duties as assigned by management.
