Job summary

Medpoint Management in Woodland Hills, CA is seeking a Medication Management Coordinator to join the Member Outreach team in QM. The role involves outreaching to members and handling inbound calls related to medication adherence, helping members obtain prescribed medications.

The coordinator will use plan data and internal reporting to target outreach, document interactions in Cozeva or sPHERICAL Solutions, coordinate with pharmacists, and support HEDIS/STARs initiatives while protecting patient

Qualifications

• High School graduate or GED equivalent.
• 2+ years working in outpatient pharmacy or managed care.
• Pharmacy Technician certification preferred (PTCE).
• Knowledge of medication names and doses.

Responsibilities

• Outreaches to members and handles inbound calls related to medication adherence.
• Uses health plan reporting to identify members needing adherence support.
• Documents calls and outcomes in Cozeva and/or Cozeva/SPhERICAL Solutions.
• Informs members about programs and routes to obtain medications.
• Promotes 100-day supplies to reduce refill frequency.
• Supports HEDIS/STARs Specialists on special projects.
