Job summary

Prime Healthcare’s Saint John Hospital in Leavenworth, KS is seeking a Medical/Surgical LPN for full-time day shifts (7:00 am – 7:00 pm). You will provide direct patient care, monitor vitals, administer medications, and support discharge planning in a fast-paced hospital setting.

Required: active state LPN license, current BLS, and IV certification; 12-hour shifts with strong teamwork and patient advocacy. Sign-on bonus available; comprehensive benefits package offered.

Qualifications

• Current and valid state LPN license.
• Current BLS certification upon hire and maintained.
• IV certification upon hire (state-specific).

Responsibilities

• Provide direct patient care, including hygiene support and mobility assistance.
• Monitor vitals, document observations, and report changes.
• Administer medications as prescribed and observe reactions.
• Perform wound care and dressing changes within LPN scope.
• Collaborate with RNs and physicians to support care plans.
• Assist with discharge planning and patient education.
• Follow infection control practices and maintain records.

Responsibilities

Essential Duties and Responsibilities (includes, but is not limited to):

• Provide direct patient care, including hygiene support, mobility assistance, and comfort measures.
• Monitor vital signs, document observations, and report changes in patient condition.
• Administer medications as prescribed and observe for reactions or side effects.
• Perform wound care, dressing changes, and assist with procedures within the state LPN scope.
• Collaborate with RNs, physicians, and the care team to support individualized care plans.
• Assist with discharge planning and provide patient/family education on treatments and recovery.
• Follow infection control practices and maintain accurate patient records.
