Job summary

Community Health Network, Inc. is seeking an experienced Medical Social Worker to support terminally ill patients and families in the Attleboro, MA area, providing compassionate home-based services as part of the interdisciplinary team.

This full-time position runs Monday–Friday, 8:00 a.m.–4:30 p.m., with no on-call or weekend requirements. MA LICSW is required, with at least 1 year in a Medicare-certified agency; hospice experience is preferred.

Qualifications

• Current MA LICSW credential is required.
• Minimum 1 year of Social Work experience in a Medicare Certified agency.
• Hospice experience preferred.
• Comfortable working in a collaborative interdisciplinary team environment.

Responsibilities

• As a member of the interdisciplinary team, the Medical Social Worker supports terminally ill patients and families.
• Plans and provides supportive services during home visits in the Attleboro, MA area.
• Reports to the Clinical Manager and participates in end-of-life care discussions.

Well established hospice agency has a need for an experienced Medical Social Worker to see patients in their homes in and around the Attleboro, MA area.

• Full-time, Monday-Friday
• 8:00 a.m. - 4:30 p.m.
• No on-call or weekends required!

As a member of the interdisciplinary team, the Medical Social Worker reports to the Clinical Manager and is responsible for planning and providing supportive services to terminally ill patients and families. Must be comfortable with issues relative to end-of life care, death and dying.




Requirements:

• Current MA LICSW
• Minimum 1 year of recent Medical Social Worker experience in a Medicare Certified agency.
  • Hospice experience preferred.
• Must be comfortable participating in a collaborative/team environment.
