Job summary

Community Health Network (CHN) is seeking an experienced Medical Social Worker to provide supportive services to terminally ill patients and families in home and facility settings around Salem, NH.

The part-time role offers 20–30 hours per week with flexible days and mileage reimbursement, reporting to the Clinical Manager as part of an interdisciplinary team. NH LICSW is required; MSW considered, hospice experience preferred; collaborative environment with competitive pay.

Qualifications

• Current NH LICSW; MSW considered.
• Minimum 1 year of recent Medical Social Worker experience in a Medicare Certified agency.
• Hospice experience preferred.
• Comfortable working in a collaborative/team environment.

Currently seeking an experienced Medical Social Worker for one our top agency partners to see patients in their homes and in facilities in Salem, Derry, Newton, Hampstead, NH and surrounding area.

• Part-time, 20-30 hours/week

As a member of the interdisciplinary team, the Medical Social Worker reports to the Clinical Manager and is responsible for planning and providing supportive services to terminally ill patients and families. Must be comfortable with issues relative to end-of life care, death and dying.




Requirements:

• Current NH LICSW; MSW considered
• Minimum 1 year of recent Medical Social Worker experience in a Medicare Certified agency.
  • Hospice experience preferred.
• Must be comfortable participating in a collaborative/team environment.
