Job summary

Hospice Services of Massachusetts is seeking a per diem social worker to support patients in the Merrimack Valley. The role offers $41 to $45 per hour with weekly pay and requires MA LSW/LICSW/LCSW licensure.

You will coordinate psychosocial care, assess client needs, and liaise with a multidisciplinary team to ensure compassionate end‑of‑life care. Responsibilities include psychosocial assessments, crisis intervention, documentation, and counseling families.

Qualifications

• Experience in health care and hospice preferred.
• Massachusetts LSW, LICSW or LCSW required.
• Must be able to read, write, speak, and understand English.
• Must possess the ability to make independent decisions when circumstances warrant such action.

Responsibilities

• Assess the psychosocial status of patients and families/c caregivers related to terminal illness and communicate findings to the nurse and interdisciplinary team.
• Carry out social evaluations including family dynamics, caregiver abilities, and high risks for suicide, neglect or abuse; plan interventions.
• Counsel patients and families as needed regarding stress and coping.
• Provide crisis intervention when necessary.
• Maintain clinical records for all patients referred to social work.
• Educate patients and families on advanced directives and assist in preparation.
• Provide information and referral services for patients and families.
• Serve as liaison between patients, families and community agencies.
• Maintain collaborative relationships with organization staff to support patient care.

$41 to $45 an hour with weekly pay.

As a Hospice Social Worker, you will plan, organize, develop, and direct all required social services to meet the needs of patients, ensuring that the medically related emotional and social needs of the resident are met/maintained on an individual basis.

Responsibilities:

• Assess the psychosocial status of patients and families/caregivers related to the patient's terminal illness and environment and communicate findings to the registered nurse and other members of the interdisciplinary group.
• Carry out social evaluations, including family dynamics, caregiver abilities, communication patterns, high risks for suicide, neglect or abuse, and plans intervention based on evaluation findings.
• Counsel patients and family/caregivers as needed in relationship to stress, and other identified coping difficulties.
• Provide crisis intervention when necessary.
• Maintain clinical records on all patients referred to social work.
• Educate patients and families on, and assist in, preparation of advanced directives.
• Provide information and referral services for organization patients and families/caregivers regarding practical and environmental needs.
• Serve as liaison between patients and families/caregivers and community agencies.
• Maintain collaborative relationships with organization personnel to support patient care.

Requirements:

• Experience in health care and hospice preferred.
• Massachusetts LSW, LICSW or LCSW required.
• An understanding of hospice philosophy and issues of death/dying preferred.
• Must be a licensed driver with an insured automobile.
• Must be able to read, write, speak, and understand the English language.
• Must possess the ability to make independent decisions when circumstances warrant such action.
• Must possess the ability to deal tactfully with personnel, residents, family members, visitors, government agencies/personnel, and the general public.
