Job summary

Pathways Healthcare, a physician- and nurse practitioner‑led home health and hospice organization, seeks a Social Worker - Hospice to join our team in Westwood, MA. The role focuses on psychosocial assessments, care planning, and coordination with the multidisciplinary team to support patients at home and in the community.

Rates are $60 per visit, with service areas including Quincy, Westwood, Brockton, Scituate and Attleboro.

Qualifications

• LICSW is required.
• End-of-life sensitivity and understanding.
• Expertise in psychosocial assessment and treatment.
• Collaborative approach with multidisciplinary team.
• At least 1 year of hospice care experience preferred.
• Strong verbal and written communication skills.
• Maintain current BLS CPR certification.
• Possess a valid driver's license and own a reliable vehicle.

Responsibilities

• Conduct psychosocial assessments related to the patient’s illness and environment and communicate findings to the registered nurse.
• Perform social evaluations and develop intervention plans based on assessment findings.
• Maintain accurate clinical records for all patients referred to social work.
• Provide information and referral services to patients, families and caregivers.
• Act as a liaison between patients, families, caregivers, and community agencies.
• Build and maintain collaborative relationships with other healthcare professionals.
• Work with public and private agencies to leverage available resources for patient benefit.
• Participate in development of comprehensive care plans and case conferences as required.
• Assist physicians and the care team in understanding social and emotional health factors.
• Contribute to discharge planning and smooth transitions for patients and families.

Job Description:

• Role: Social Worker - Hospice
• Rate: $60/visit
• Main Office location: Westwood, MA
• Service Area: Quincy, Westwood, Brockton, Scituate, Attleboro areas.
