Job summary

Hospice Services of Massachusetts is seeking a per diem social worker to support our patients in the Merrimack Valley. The role focuses on planning, organizing, developing, and directing social services to meet the emotional and social needs of residents at the end of life.

You will assess psychosocial status, provide counseling, coordinate with the interdisciplinary team, maintain records, educate families on advance directives, and connect patients with community resources.

Qualifications

  • Experience in health care and hospice preferred.
  • Massachusetts LSW, LICSW or LCSW required.
  • Must be able to read, write, speak, and understand English.
  • Ability to make independent decisions when warranted.

Job description

Hospice Services of Massachusetts is looking for a per diem social worker to support our patients in the Merrimack Valley.

$41 to $45 an hour with weekly pay.

As a Hospice Social Worker, you will plan, organize, develop, and direct all required social services to meet the needs of patients, ensuring that the medically related emotional and social needs of the resident are met/maintained on an individual basis.

Responsibilities
  • Assess the psychosocial status of patients and families/caregivers related to the patient's terminal illness and environment and communicate findings to the registered nurse and other members of the interdisciplinary group.
  • Carry out social evaluations, including family dynamics, caregiver abilities, communication patterns, high risks for suicide, neglect or abuse, and plans intervention based on evaluation findings.
  • Counsel patients and family/caregivers as needed in relationship to stress, and other identified coping difficulties.
  • Provide crisis intervention when necessary.
  • Maintain clinical records on all patients referred to social work.
  • Educate patients and families on, and assist in, preparation of advanced directives.
  • Provide information and referral services for organization patients and families/caregivers regarding practical and environmental needs.
  • Serve as liaison between patients and families/caregivers and community agencies.
  • Maintain collaborative relationships with organization personnel to support patient care.
Requirements
  • Experience in health care and hospice preferred.
  • Massachusetts LSW, LICSW or LCSW required.
  • An understanding of hospice philosophy and issues of death/dying preferred.
  • Must be a licensed driver with an insured automobile.
  • Must be able to read, write, speak, and understand the English language.
  • Must possess the ability to make independent decisions when circumstances warrant such action.
  • Must possess the ability to deal tactfully with personnel, residents, family members, visitors, government agencies/personnel, and the general public.
