Caringhand Home Health Solutions, Inc. is seeking a Medical Social Worker to lead our social services team in California. You will mentor social workers, oversee care planning, and ensure compliance with ACHC and regulatory standards.

This field-based, per diem role requires travel to patients’ homes. A California LCSW is required along with an MSW from an accredited program and 3+ years of management experience. Possibility to convert to full-time based on performance.

Qualifikationen

• MSW from an accredited program.
• Active California license as an LCSW.
• At least 3 years of management experience, preferably in home care.
• Strong knowledge of healthcare regulations, ACHC standards, and business operations.

• Lead daily operations of our social services team, including policies, procedures, and care coordination with other clinical providers.
• Recruit, onboard, and mentor social workers—providing clinical supervision, observing practice, and supporting their professional growth.
• Oversee assessment, planning, and evaluation of social services care, and ensure clinical documentation meets state, federal, and ACHC standards.
• Partner with the Executive Director on the annual department budget and resource planning.
• Build knowledge of community resources and connect patients and referral sources to the right support.
• Plan in-service training and continuing education to keep the team’s skills sharp.
• Contribute to quality improvement initiatives and help identify new ways to better serve our patients.
• Perform other duties as required for patient services.

This is a field-based, per diem position that requires travel to patients’ homes. Although listed as “remote,” this is not a work-from-home or telehealth role. Compensation is $100-$120 per visit.
