Avance Care in Durham, NC is hiring a Referral Coordinator to support providers by submitting referrals to high-value specialists and closing the loop with consult notes after visits.

This full-time role offers a hybrid schedule (3 days remote, 2 days onsite) with weekdays only shifts, a comprehensive benefits package, and a fast-paced environment ideal for detail-oriented candidates with healthcare admin experience.

Qualificações

• High School Diploma or associate degree in healthcare administration preferred.
• Medical terminology knowledge preferred.
• 1 year of related experience preferred.
• Strong computer skills and HIPAA confidentiality required.
• Excellent verbal and written communication; detail-oriented and organized.

Responsabilidades

• Ensure referrals are submitted, accepted and followed up promptly.
• Provide clinical information to outside entities as needed.
• Ensure complete and accurate registration for patients.
• Assist patients with problems related to referrals and advocate with carriers for approvals.
• Maintain referral documentation and communication with referring providers in the EMR.
• Establish and maintain relationships with providers and outside entities.
• Review referral details with patients as needed.
• Keep patient charts up to date with consult information from other entities.
• Monitor utilization of High Value Specialist and Medical Neighborhood networks.
• Communicate with specialist offices to coordinate transitions of patient care.

As aReferral Coordinator, you’ll aid providers with continuity of care by ensuring referrals get submitted promptly to high value specialists and close the loop of care by obtaining consult notes after specialist visits.

This is a full-time role involving8 hours weekday shiftswith no weekends schedule.

• The position works a hybrid schedule with 3 days remote and 2 days onsite.

Selected Responsibilities:

• Ensure referrals are submitted, accepted and followed up on in a timely manner
• Provide appropriate clinical information to outside entities, as needed
• Ensure complete and accurate registration for patients
• Assist patients in problem solving issues related to their referrals including taking an advocate role on the patient’s behalf with the carrier to ensure approval of necessary services in a timely fashion
• Maintain detailed and accurate referral documentation and communication with referring providers within the electronic medical record
• Establish and maintain relationships with providers and outside entities
• Review details and expectations about the referral with patients as neededi>
• Ensure patient’s chart is up to date with consult information from other entities
• Monitor and ensure proper utilization of the High Value Specialist and Medical Neighborhood networks so patients have access to the right care at the right tim
• Communicate effectively with specialist offices assuring safe and coordinated transitions of patient care

Candidates should have a High School Diploma, or associate’s degree in healthcare administration or related field, preferred. Along with medical terminology knowledge and 1-year related experience preferred.
