Illumination Health & Home is seeking a Manager of Medical Recuperative Care in California to oversee discharge planning for homeless patients, coordinate medical follow-up, and lead social care across recuperative centers and shelters.

The role is full-time, with a Monday–Friday 8:00 a.m. – 4:30 p.m. schedule. Responsibilities include staff supervision, community relations, compliance, and administration, with a salary of $80,000 per year.

Formación

• High School Diploma or equivalent.
• Current and non-restricted LVN or RN license in California.
• 2 years of relevant experience.
• Valid CA Driver’s License.

Responsabilidades

• Oversee discharge care plan for homeless patients.
• Coordinate medical follow-up and social care for clients.
• Oversee Medical Coordination Department operations and standards.
• Maintain client data and documentation for ongoing care and billing.
