Job summary

Illumination Health & Home is seeking a Manager of Medical Recuperative Care to oversee discharge planning and ensure seamless medical follow-up for homeless patients across recuperative care centers. The role emphasizes social care coordination, community relations, and adherence to operational standards.

The position requires a California LVN/RN license and at least 2 years of relevant experience, with strong communication and leadership skills to supervise the Medical Coordination department

Qualifications

• Bilingual in English and Spanish is preferred.
• Proficiency with Google Suite and basic EHR usage.
• Proficiency with KIPU or other electronic health record systems.

Responsibilities

• Oversee and optimize the discharge care plan for homeless patients.
• Coordinate high-quality medical follow-up and social care in recuperative care centers.
• Supervise and train staff within Medical Coordination departments; ensure compliance and reporting.

Skills

Education

High School Diploma or equivalent
