Qualifications

• High School Diploma or equivalent.
• Current and non‑restricted LVN or RN license in California
• 2 years of relevant experience.
• Basic computer skills, including email and spreadsheets.
• Valid CA Driver’s License.

Responsibilities

• Oversee discharge care plans for homeless patients from hospitals or contracted agencies.
• Coordinate high‑quality medical follow‑up and social care.
• Supervise Medical Oversight and Monitoring of the Medical Coordination Department.
• Maintain documentation and data collection for client services.
• Ensure compliance with HIPAA guidelines and organization policies.
