Illumination Health & Home is seeking a Manager of Medical Recuperative Care in California to oversee discharge plans from hospitals and coordinate medical follow-up and social care. This role ensures regulatory compliance and high-quality care across Recuperative Care Centers.

The position offers a salary of $80,000 per year with a Monday–Friday schedule, 8am–4:30pm, to support patients’ health and housing stability within IH+H’s service footprint.

Qualifikationen

• High School Diploma or equivalent is required.
• Current and unrestricted LVN or RN license in California.
• Minimum 2 years of relevant experience in a healthcare setting.
• Basic computer skills and ability to summarize data in spreadsheets.
• Valid California Driver’s License.

Aufgaben

• Oversee and optimize discharge care plans for homeless patients.
• Coordinate high-quality medical follow-up and social care.
• Lead and supervise Medical Coordination across locations.
• Maintain documentation and data in KIPU and other systems.
• Develop community relations and liaise with service providers.
