Job summary

Clarion Psychiatric Center seeks a Medical Records Clerk to manage and maintain patient health records, ensuring accuracy, confidentiality, and accessibility for providers. You will retrieve records for providers, patients, external sources, and staff, and ensure HIPAA compliance across all processes.

The role involves charting, coding using ICD-10-CM and DSM‑V, and abstracting, with emphasis on confidentiality and security of information in a hospital setting.

Qualifications

• High school graduate with Medical Records/HIM experience; completion of a Medical Coding course and CPC certification.
• Scribe duties require a valid nursing license to dictate discharge summaries.
• Three years of Health Information Management experience; DSM-V and ICD-10-CM coding knowledge.
• Knowledge of medical and psychiatric terminology; ability to abstract charts; proficient in MS Office.
• Hospital setting; lifting up to 50 lbs; potential for crisis situations.

Responsibilities

• Chart assembly and analysis; ICD-10-CM and DSM‑V coding; chart abstraction.
• Maintain organized Medical Records Department; assist with performance improvement studies.
• Filing; record review; respond to internal and external inquiries.
