Job summary

Colorado PACE is seeking a Medical Director to oversee medical care, quality assessment, and performance improvement for participants across two Colorado locations. The role requires engagement with the Interdisciplinary Team, home and clinic evaluations, and leadership in clinical practice.

The position offers full-time hours, a Monday–Friday schedule, and a strong benefits package including substantial PTO, retirement matching, and education reimbursements. Relocation support is provided.

Qualifications

• Graduate from approved school of medicine.
• Board certified or board eligible in Internal Medicine, Family Practice, Hospice & Palliative Medicine.
• Colorado medical license or eligibility.

Responsibilities

• Provide comprehensive medical care to PACE participants and oversee care plans.
• Lead the IDT and participate in regular care planning and case reviews.
• Evaluate participants in clinic, home, and inpatient settings as needed.
• Collaborate on individualized care plans and provide disease-process guidance.
• Maintain timely documentation in the EMR and participate in on-call rotation.
