Job summary

Curative seeks a collaborative, mission-driven physician to join a geriatric care model for medically complex, dual-eligible seniors, helping them remain safe at home and in their communities.

You’ll start at the flagship site with full training and panel development, then move to a Denver center with a clear path to leadership as Associate Medical Director.

This deeply supported, team-driven environment emphasizes high-touch, relationship-based care with strong IDT collaboration.

Qualifications

• MD or DO with Internal Medicine or Family Medicine residency.
• Fellowship in Geriatrics or Hospice & Palliative Medicine strongly preferred.
• Comfort managing medically complex, frail, dual-eligible older adults.
• Interest in SNF-based care, PACE models, or safety-net populations.

Responsibilities

• 1–5 daily face-to-face encounters with complex older adults.
• Extended visits focused on comprehensive care coordination.
• Territory coverage across seven SNFs in Boulder and Weld Counties.
• Full participation in interdisciplinary team (IDT) huddles and care planning.
• 100% telephone-based call from home with nurse triage, fully compensated.
