Aunt Martha's Health & Wellness is seeking a Medical Case Management Care Coordinator in Aurora, IL. This onsite role supports timely delivery of case management services for youth, coordinating care with caregivers, caseworkers, and providers.

The ideal candidate will manage documentation, reviews, and communications while meeting funder benchmarks. Growth opportunities exist within HealthWorks for advancing to leadership roles.

Qualifications

• Bachelor's or Associate degree in Social Work or related field.
• Valid Illinois driver's license and current automobile insurance.
• Strong written and verbal communication skills.
• Proficiency with databases, Excel, and Access.
• Excellent organizational skills and ability to manage deadlines.
• Experience with case management or child welfare preferred.

Responsibilities

• Coordinate medical case management activities within contractual timeframes.
• Review case documentation and request medical records.
• Prepare and distribute case management packets to caseworkers.
• Maintain accurate data in HealthWorks Evolve Portal.
• Generate production reports for the Director.
