The role emphasizes organized, detail-oriented work with data entry in the HealthWorks Evolve Portal, adherence to timelines, and use of Excel/Access. Competitive hourly pay and benefits are offered.

Qualifications

• Bachelor's or Associate degree in social work, computer field, or related discipline, or equivalent combination of education and experience.
• Valid Illinois driver's license and current automobile insurance.
• Strong written and verbal communication skills.
• Proficiency with databases, Excel, Access, and other computer applications.
• Strong organizational skills with ability to manage multiple priorities and deadlines.
• Ability to analyze information, solve problems, and maintain accurate documentation.

Responsibilities

• Ensure required medical case management activities are completed within established contractual time frames.
• Assist with the final review of completed critical case activities.
• Monitor upcoming medical examinations, ACRs, and other required services per funder benchmarks.
• Complete individualized treatment plans for assigned cases.
• Follow up on pending assignments, documentation, and barriers affecting timely service delivery.
• Contact primary care and dental providers to request medical records.
• Review received health information for completeness and accuracy.
• Prepare and distribute Medical Case Management packets to assigned caseworkers timely.
• Coordinate communication with healthcare providers to ensure continuity of care.
• Generate correspondence to notify caregivers and caseworkers of providers.
• Collaborate with caregivers, providers, and caseworkers to support coordinated care.
• Complete accurate data entry within HealthWorks Evolve Portal.
• Maintain complete case notes for all assigned cases.
• Review SACWIS and DCFS reports.
• Monitor daily/weekly/monthly reports to ensure contractual benchmarks.
• Generate daily production reports for the Director.
