Job summary

Aunt Martha's Health & Wellness is seeking a Medical Case Management Care Coordinator in Park Forest, IL. You will support youth in obtaining timely healthcare and coordinating essential case management services.

Responsibilities include monitoring services, coordinating with providers and caregivers, and maintaining documentation in the HealthWorks system. This role offers full-time hours, onsite work, and opportunities for growth within a mission-driven team.

Qualifications

• Bachelor's or Associate degree in Social Work, a computer-related field, or a related discipline, or an equivalent combination of education and experience.
• Valid Illinois driver's license and current automobile insurance.
• Strong written and verbal communication skills.
• Proficiency with databases, Microsoft Excel, Microsoft Access, and other computer applications.

Responsibilities

• Coordinate medical records and packets for caseworkers within required timeframes.
• Monitor contractual time frames for required case management activities and reviews.
• Communicate with caregivers, caseworkers, and healthcare providers to support coordinated care.
• Maintain accurate case notes and data in the HealthWorks Evolve Portal.
• Review SACWIS and other DCFS reports and generate daily production reports for the Director.
