Job summary

Aunt Martha’s Health & Wellness, Inc. is seeking a Behavioral Health Care Manager in Aurora, IL. You will work with medical providers and a Psychiatric Consultant to manage a caseload, deliver brief evidence-based interventions, and coordinate comprehensive behavioral and physical health care.

The role emphasizes patient engagement, monitoring outcomes, and connecting patients to community resources within a Collaborative Care Model.

Qualifications

Required licensure in respective clinical field.
Demonstrated experience in behavioral health care coordination.
Ability to work within an interdisciplinary healthcare team.
Strong patient-engagement and communication skills.
Experience with treatment planning and outcomes monitoring.

Responsibilities

Screen and assess patients for mental health and substance use disorders.
Engage patients in treatment and provide ongoing follow-up care.
Educate patients about behavioral health conditions and treatment options.
Coordinate behavioral and physical healthcare with medical providers and consultants.
Participate in caseload consultations with the Psychiatric Consultant.
Communicate treatment recommendations to the care team.
Assist with treatment-plan changes when needed.
Facilitate referrals to specialty behavioral health services.
