Illumination Health + Home in Los Angeles County is seeking a Manager of Care Coordination & Case Management to oversee day-to-day operations, supervise case managers, monitor budgets, and implement policies in the Recuperative Care Department.

This role runs Monday–Friday, 9am–5:30pm, with an annual salary range of $75k–$78k, reflecting commitment to client care, collaboration across programs, and driving outcomes toward self-sufficiency.

Qualifications

• Masters Degree in Social Services with 2 years supervisory experience.
• Bachelor’s degree in Social Services or related field.
• At least 2 years supervisory/admin experience in social services.
• Experience with case management or similar role.
• CA driver's license and dependable vehicle.

Responsabilités

• Supervise case management staff and oversee daily tasks.
• Monitor budgets and help develop policies and procedures.
• Coordinate client care with program and site managers.
• Ensure data accuracy for contract compliance and reporting.
• Represent the program in community meetings and partnerships.
