Medical Assistant II - Accountable Care Organization

This is a part-time 20 hours per week position.

The ACO Medical Assistant is a vital member of the multidisciplinary Care Management team, supporting preventative care and chronic condition outreach programs for D-H attributed patients. The role involves patient engagement, EMR utilization, and acting as a liaison to improve patient outcomes and meet performance targets.

Responsibilities

• Partner with patients and care management team to achieve shared goals.
• Conduct patient outreach via phone, portal messaging, or mail to schedule appointments and screenings.
• Document patient information accurately in the EMR.
• Monitor patient outcomes related to screenings and chronic disease management.
• Handle patient interactions, identifying barriers to care, and involve nursing or leadership as needed.
• Review care gaps and disease registries for outreach opportunities.
• Provide authorized information to third parties upon request.
• Assist patients with completing care plan forms.
• Coordinate pre-visit planning, reminders, and instructions.
• Perform additional duties as assigned.

Qualifications

• Graduate of an accredited Medical Assistant Program.
• Proficient in Microsoft Office (Word, Excel, Outlook, PowerPoint).
• Excellent communication and interpersonal skills.
• Strong organizational and time management skills.
• Ability to work independently and in a team.
• Preferred: Outpatient care experience, support for clinical quality programs (e.g., HEDIS, Care Coordination).

Licensure/Certifications

• Certified Medical Assistant (CMA), Registered Medical Assistant (RMA), or Certified Clinical Medical Assistant (CCMA).
• Current BLS certification.
