Qualifications

• Experience with EPIC or similar EHR/billing systems.
• CPR certification is required.

Responsibilities

• Performs ED/IP admissions and discharge follow-up; views EPIC and CRISP medical records to assist in care plans.
• Conducts Social Determinant of Health patient screenings.
• Assists with care coordination of Medicare beneficiaries.
• Maintains logs of preventative care (colonoscopies, diabetic eye exams, mammograms).
• Submits insurance requests for medications and tracks authorizations.
• Enrols eligible patients with long term conditions in chronic care management.
• Assists nursing staff with medication refills and patient education.
• Schedules follow-up appointments and specialist visits.
