Job summary

The Excalibur Group in Washington, DC is seeking a Support Broker/Case Manager to coordinate daily Medicaid Waiver services and promote the independence of participants. This full-time role emphasizes advocacy, plan coordination, and community integration.

The position is remote with regular customer-site visits. A background check is required, and the role follows GT Independence policy and procedures to ensure quality service.

Qualifications

• Must have a Master’s degree in social work, psychology, counseling, rehabilitation, nursing gerontology or sociology and 1 year experience; OR a Bachelor’s degree with 2 years experience; OR RN/Associate degree with 3 years experience.

Responsibilities

• Coordinate daily operation of Medicaid Waiver services.
• Promote personal growth and independence of participants.
• Develop and maintain relationships with agency staff and families.
• Ensure quality documentation and adherence to regulations.
• Support planning and execution of ISP/IEP and related plans.
• Provide guidance on budget and employer of record models.
• Coordinate with financial and community resources.
• Monitor safety, ethics, and compliance with standards.
• Maintain flexible work hours and travel as needed.
• Communicate effectively with participants and supports.
