Job summary

GT Independence, LLC is seeking a Case Manager Supervisor (Support Broker Lead) in Silver Spring, MD to lead a team delivering Medicaid Waiver services. The role combines supervision with a caseload to ensure program rules and metrics are met, promoting participant growth and independence.

You will train staff on self-directed options, manage budgets, and coordinate with providers to support individuals. This is a hybrid, full-time position in the Maryland area.

Qualifications

• Meet the DHCF requirements for a waiver case manager.
• Have current, appropriate licensure.
• Master degree in Social Work, Psychology, Counseling, Rehabilitation, Nursing Gerontology or Sociology and one year experience with the population.
• Bachelor degree and two years’ experience with the population.
• Registered Nurse (RN) or can have an Associate degree and three years’ experience with the population.
• Have at least two years of management or supervisory experience.

Responsibilities

• Supervise and manage day to day operations for a team of support brokers as well as carry a caseload, as appropriate.
• Ensure all program rules are adhered to by their team as well as all operational metrics are achieved.
• Ensure personal growth and independence of service participants through initial development and on-going support, monitoring, supervision, and training of staff.
• Develop and maintain relationships with Agency staff, other providers and individuals/families.
• Provide training on the Individual and Family Directed supports Option, if the participant is considering directing services and supports (Employer of Record Model).
• Provide guidance with the management of the individual & Family directed budget (Employer of Record Model).
• Coordinate services with the financial Support Services provider, if the individual is self-directing services under the Employer of Record Model, including guidance on use of the Individual and Family Directed (Participant-Directed Budget) (Employer of Record Model).
• Provide information on recruiting, hiring, managing, training, evaluating, and changing support staff, if the participant is self-directing services (Employer of Record Model).
• Assist with the development of schedules and outlining staff duties, if the participant is self-directing services considering directing services and supports (Employer of Record Model)
• Assist with understanding staff financial forms, qualifications and record keeping requirements, if the participant is self-directing services (Employer of Record Model)
• Inform and coordinate community resources including coordination among primary, preventative and chronic care providers.

Education

Master degree in Social Work, Psychology, Counseling, Rehabilitation, Nursing Gerontology or Sociology
