Job summary

GT Independence is seeking a dedicated Support Broker to coordinate Medicaid Waiver services and promote participant independence in San Francisco, CA. The role emphasizes collaboration with GT staff, community partners, and families, ensuring compliant documentation and successful program outcomes.

The position requires relevant licensure or advanced degree, strong communication, and the ability to travel for events and consultations with regional centers.

Qualifications

• Master's degree or higher in social work, psychology, counseling, rehabilitation, nursing gerontology or sociology.
• Bachelor's degree plus two years’ relevant experience required.
• RN or Associate degree with three years’ experience may qualify.
• Must meet required criminal background checks and licensure criteria.

Responsibilities

• Promote growth, independence, and quality of life for participants.
• Build relationships with staff, providers, families, and participants.
• Ensure documentation meets state, agency, and HCBS requirements.
• Travel to events, conferences, and training as needed.
• Provide training and guidance to staff and participants on ISP outcomes.

Education

Master's degree in social work, Psychology, Counseling, Rehabilitation, Nursing Gerontology or Sociology

Bachelor's degree and two years’ experience with the population

Registered Nurse (RN) or Associate degree with three years’ experience
