Job summary

Queens Boulevard Extended Care Facility seeks a Medicaid Coordinator to help clients apply for and renew Medicaid, manage eligibility recertifications, and coordinate with providers and agencies to ensure continuous coverage. You will handle documentation, stay compliant with NYS/NYC regulations, and educate families on benefits.

The role involves liaising with health plans, social services, and government entities, maintaining records, and resolving issues to ensure access to services.

Qualifications

• Experience with Medicaid/public benefits preferred.
• Knowledge of NYS Medicaid and NYC HRA processes.
• Strong organizational and documentation skills.
• Bilingual abilities are a plus.

Responsibilities

• Assist clients with Medicaid applications, renewals, and changes.
• Determine eligibility based on NYS/NYC guidelines.
• Submit documentation to NYC HRA and NY State of Health.
• Monitor case statuses and follow up as needed.
• Coordinate with providers, plans, and social services agencies.
• Educate clients on benefits and options.
• Maintain accurate records in CMS.
• Resolve coverage issues via appeals or actions.
