Job summary

NYREACH is seeking a Certified Application Counselor to support Medicaid recertification, enrollment renewals, and coverage continuity for Bronx residents. The role focuses on guiding patients through renewal cycles, educating on plan options, and ensuring compliant documentation within NYSOH and EHR systems.

The position requires an associate degree with NYSOH Assistor Certification or equivalent credentials, bilingual Spanish/English, and strong communication skills for outreach and scheduling

Qualifications

• Bachelor-level or associate degree required (or equivalent experience).
• Knowledge of NYSOH processes and healthcare enrollment basics preferred.
• Bilingual in Spanish and English preferred; strong communication for diverse populations.

Responsibilities

• Assist Medicaid recertification and renewals for enrolled clients.
• Navigate NYSOH enrollment portal to submit applications and renewals.
• Educate clients on Medicaid, Essential Plan, and QHP options and out-of-pocket costs.
• Document patient interactions and maintain HIPAA-compliant records.
• Meet monthly renewal and submission targets with accurate, timely documentation.
