Job summary

Responsibilities include home visits (75-90% travel), coordinating services, and guiding members through benefits and eligibility. Requires relevant healthcare experience and Illinois residency, with a Monday–Friday schedule and travel readiness.

Qualifications

• The Care Coordinator must meet one of the listed degree or license options.
• Experience in health care and/or case management is required.
• Experience with Medicare & Medicaid recipients and/or long-term care is required.
• Intermediate to advanced computer skills and EMR experience.

Responsibilities

• Visit Medicaid members in homes or care facilities and coordinate services.
• Assess needs and develop personalized care plans.
• Identify and resolve barriers to care and monitor progress.
• Coordinate services and evaluate progress toward goals.
• Assist with grievances and appeals and provide program information.
