CVS Health is hiring for Engagement Outreach Hub Care Management Associates to support Medicaid members through telephonic outreach and enrollment into care management. You’ll blend sales, navigation and customer support skills to connect members with care managers and help implement care plans.

The role emphasizes efficient outreach, accurate documentation, and compliance while delivering compassionate service. Full-time hours include 40 per week with benefits.

Qualifications

• Ability to meet daily metrics with speed, accuracy and a positive attitude.
• Effective communication, telephonic and organization skills with ability to multitask.
• Strong customer service skills to coordinate service delivery while ensuring privacy and compliance.
• Documentation of each member call in the electronic record with detail.
• Ability to work independently and in a multi-disciplinary team.

Responsabilités

• Conduct telephonic outreach to Medicaid members and enroll them into care management.
• Facility case assignment and connect identified members to care managers via warm transfer.
• Document each interaction accurately in the electronic record to ensure compliance.
• Support team members by answering calls, researching information and problem solving.
• Maintain confidentiality and adhere to regulatory requirements.
