CVS Health via Aetna Medicaid Care Management Engagement Outreach Hub seeks a Care Management Associate to conduct telephonic outreach for eligible Medicaid members. You will enroll members into care management, support care plan implementation, and connect them with care managers as needed while documenting interactions in secure records.

The role emphasizes dynamic interaction, compliance with privacy, and collaborative work in a fast-paced environment.

Qualifications

• Demonstrates ability to meet daily metrics with speed, accuracy and a positive attitude.
• Effective communication and organization skills; able to manage multiple priorities and adapt to change.
• Strong customer service skills to coordinate service delivery, protect privacy, and ensure compliance.

Responsabilités

• Engage in direct member outreach and enrollment into care management.
• Document each member call in the electronic record with high detail.
• Resolve issues and coordinate with care managers via warm transfers.
• Meet deliverables and deadlines while maintaining an enthusiastic, caring presence telephonically.
