Job summary

Glenwood Skilled Nursing & Therapy is seeking a PPS Skilled Case Manager / MDS Coordinator (RN or LPN) to join our Nursing team in Glenpool, OK. This full-time day position combines skilled case management with the complete MDS process to support accurate documentation, regulatory compliance, and quality resident outcomes.

The role involves coordinating the MDS cycle, care planning, and payer requirements, along with timely data submission and interdisciplinary collaboration.

Qualifications

• Current Oklahoma RN or LPN license.
• Associate degree with nursing licensure or BSN.
• Knowledge of MDS/RAI process and long-term care requirements.
• Ability to develop comprehensive plans of care.
• 2+ years MDS/long-term care or acute case management experience preferred.
• First Aid and CPR certification.
• Basic computer skills including Microsoft Office.
• Strong organizational, communication, and documentation skills.

Responsibilities

• Coordinate complete MDS process: scheduling, data collection, completion, review, and electronic submission.
• Complete and coordinate required MDS assessments on schedule.
• Ensure MDS reflects resident condition and care needs from records.
• Maintain MDS and Care Plan tracking for timely completion and compliance.
• Coordinate development of individualized care plans.
• Gather clinical information for assessments and payer requirements.
• Coordinate payer requirements with resident needs and treatment plans.
• Monitor documentation to reflect services in the medical record.
• Communicate with nursing, therapy, providers, and interdisciplinary team.
• Ensure documentation aligns with state and federal regulations.
• Record and transmit MDS data to state and federal databases.
