FullShift Staffing is partnering with a dedicated Skilled Nursing Facility to hire an MDS Coordinator (RN/LPN). You will lead accurate MDS assessments, ensure compliant case-mix documentation, and collaborate with clinical teams to optimize resident care and reimbursement.

Qualifications

• RN or LPN licensure in the appropriate state.
• Strong knowledge of the MDS process, care planning, and regulatory compliance.
• Excellent organizational, communication, and time management skills.

Responsibilities

• Coordinate timely, accurate assessment of residents' health and function.
• Ensure completion of Medicare/Medicaid case-mix documents for appropriate reimbursement.
• Track MDS-related cases using case management tools and coordinate with Clinical Reimbursement Specialist and other teams.
• Collaborate with interdisciplinary teams to develop and manage care plans.
• Ensure timely submission of all MDS assessments and related documentation.
