Job summary

Southwest LTC in Oklahoma City seeks an experienced MDS Coordinator to manage the nursing aspects of the RAI MDS, HCFA Medicare, DADS and RMHP programs, including screening for skilled care and pre-admissions.

You will coordinate MDS schedules, ensure accurate documentation, work with the interdisciplinary team, and support timely reporting and billing through MDS and RUG processes in a busy long-term care setting.

Qualifications

• RN or LVN with valid state license.
• Experience in long-term care preferred.
• Medicare/MDS knowledge encouraged.

Responsibilities

• Screen new admits for skilled care and facility policies.
• Coordinate MDS schedules across clinical disciplines.
• Ensure MDS documentation is encoded, edited and transmitted timely.
• Collaborate with interdisciplinary team on care plans and discharge planning.
• Support Medicare and RUG processes and look-back periods.
