The ideal candidate has Oklahoma licensure and 2+ years in MDS or case management, with strong interdisciplinary collaboration and regulatory knowledge.

Qualifications

• Current and unencumbered Oklahoma RN or LPN license.
• 2+ years of MDS, long-term care, skilled nursing, or acute case management experience preferred.
• Knowledge of MDS, RAI, care planning, and long-term care requirements.
• Ability to develop comprehensive, individualized care plans.

Responsibilities

• Coordinate and oversee the complete MDS process, including scheduling, data collection, completion, review, and electronic submission.
• Complete and coordinate required MDS assessments, accurately and within required timeframes.
• Ensure MDS assessments reflect residents' medical condition and care needs.
• Maintain MDS and Care Plan tracking for timely completion and compliance.
• Coordinate development of individualized, comprehensive care plans.
• Gather clinical information needed for skilled assessments and payer requirements.
• Coordinate payer requirements with resident needs and treatment plans.
• Monitor documentation to ensure services are reflected in the medical record.
• Communicate with nursing, therapy, providers, and the treatment team regarding needs and planning.
• Electronically record and transmit MDS data to state/federal databases.
• Collaborate with the interdisciplinary team to promote quality outcomes and resident-centered care.

PPS Skilled Case Manager / MDS Coordinator

Woodward Skilled Nursing & Therapy is actively seeking a detail-oriented and knowledgeable licensed nurse (RN or LPN) to join our team as a PPS Skilled Case Manager / MDS Coordinator!

This combined role is responsible for both skilled case management and the complete MDS process. The PPS Skilled Case Manager / MDS Coordinator will coordinate resident assessments, care planning, payer requirements, and MDS completion to support accurate documentation, regulatory compliance, appropriate reimbursement, and quality resident outcomes.

Responsibilities and Purpose

• Coordinate and oversee the complete MDS process, including scheduling, data collection, completion, review, and electronic submission of assessments in accordance with applicable state and federal requirements.
• Complete and coordinate required MDS assessments, including quarterly and other scheduled assessments, accurately and within required timeframes.
• Ensure MDS assessments accurately reflect each resident’s current medical condition, functional status, and care needs based on assessments and medical record documentation.
• Maintain accurate MDS and Care Plan tracking to ensure timely completion and regulatory compliance.
• Coordinate the development and completion of individualized, comprehensive resident care plans.
• Oversee the gathering of clinical information needed for skilled assessments, care planning, and payer requirements.
• Coordinate payer requirements with resident needs, treatment plans, and desired clinical outcomes.
• Monitor skilled residents and supporting documentation to help ensure services and care provided are appropriately reflected in the medical record.
• Communicate effectively with nursing, therapy, providers, and other members of the interdisciplinary treatment team regarding resident needs, documentation, and care planning.
• Participate in care plan decisions and ensure required documentation is completed in accordance with state and federal regulations.
• Electronically record and transmit MDS data to the appropriate state and federal databases as required.
• Work cooperatively with the interdisciplinary team to promote quality outcomes and compassionate, resident-centered care.

Experience and Education

• Current and unencumbered Oklahoma Registered Nurse (RN) or Licensed Practical Nurse (LPN) license.
• 2+ years of experience with MDS, long-term care, skilled nursing, or acute case management preferred.
• Knowledge of the MDS, RAI process, care planning, and applicable long-term care requirements.
• Ability to develop comprehensive, individualized plans of care based on residents’ current conditions and care needs.
