Job summary

Gilder Search Group is seeking an MDS Coordinator (RN or LPN) for our Skilled Nursing Facility in Baltimore, MD. You will own the resident assessment process from admission through discharge, complete and transmit MDS on schedule, and lead care plan meetings with the interdisciplinary team.

This full-time, on-site role requires Maryland RN or LPN licensure, knowledge of MDS 3.0 and PDPM, and strong documentation and computer skills. Competitive pay and benefits are offered.

Qualifications

• Must hold Maryland RN or LPN license.
• Knowledge of MDS 3.0 assessment process and PDPM.
• Experience in a skilled nursing or long-term care setting preferred.
• Strong documentation and computer skills.
• Able to work full-time on site in the building.

Responsibilities

• Complete, code and transmit MDS assessments within CMS timeframes
• Schedule and track the assessment calendar for all residents
• Lead care plan meetings with nursing, therapy, dietary and social services
• Review documentation for accuracy and reimbursement support
• Serve as the building resource for resident assessment questions
