Job summary

OPCO Skilled Management is seeking an experienced MDS Coordinator in Garland, Texas. The role supports the Director of Nursing and RN Assessment Coordinator to ensure documentation meets federal, state, and certification guidelines, and coordinates the RAI process for MDS, CAAs, and the care plan.

The position requires an RN or LVN license in good standing and at least 2 years of nursing experience in a Skilled Nursing Facility, with strong knowledge of Case-Mix, Medicare PPS, Medicaid

Qualifications

• RN or LVN license required and current in-state practice.
• Minimum of 2 years nursing experience in a Skilled Nursing Facility preferred.
• Thorough knowledge of Case-Mix, Medicare PPS, Medicaid reimbursement, and the OBRA regulations.

Responsibilities

• Assist Director of Nursing and RN Assessment Coordinator to ensure documentation meets Federal, State, and Certification guidelines.
• Coordinate the RAI process to ensure timely and complete MDS, CAAs, and Interdisciplinary Care Plan.
• Participate in regulatory or certification surveys and address deficiencies as needed.
• Report trends from audits to the Quality Assurance Committee.
• Ensure accurate determination of Assessment Reference Date reflecting patient care needs and resources.
