Sequoia Vista at Kaweah Manor seeks an RN/LVN to oversee timely MDS completion and care management from admission to discharge. You will coordinate with the Director and VP to ensure compliance with Federal, State, and Certification guidelines.

The role requires knowledge of MDS 3.0, care planning, and data entry, with experience in Skilled Nursing Facilities preferred. Maintains MDS status and supports interdisciplinary collaboration.

Qualifikationen

• RN or LVN license in the state of practice is required.
• Minimum of 2 years nursing experience in a Skilled Nursing Facility preferred.
• Knowledge of MDS 3.0, PPS/Medicaid reimbursement, OBRA regulations.
• Understanding of the care planning process and data entry into MDS systems.

Aufgaben

• Collaborate with Interdisciplinary Team to schedule MDS assessments and care plan reviews.
• Ensure decisions on MDS and assessments are based on clinical criteria per MDS User's Manual.
• Coordinate daily stand-up meetings; review resident care and assessment reference dates.
• Maintain MDS data entry accuracy and current MDS status per regulations.
• Adhere to federal and state regulations regarding completion and coordination of the RAI process.
