Job summary

HealthSource Saginaw is seeking an MDS Registered Nurse for our Long Term Care program. The role includes overseeing MDS assessments, care planning, and discharge planning with interdisciplinary teams.

Qualified candidates hold an RN license in Michigan and have experience in MDS/RAI and utilization management; RAC certification is preferred or obtainable within 12 months. This is a Full-Time, on-site opportunity with comprehensive benefits.

Qualifications

• RN licensed in Michigan with strong clinical and technical expertise in MDS/RAI and utilization management.
• Experience in case management and discharge planning; knowledge of third-party reimbursement.
• RAC certification or willingness to obtain within 12 months of hire.

Responsibilities

• Coordinate and oversee the MDS assessment process and regulatory compliance.
• Complete and manage MDS assessments per federal/state guidelines and facility policies.
• Collaborate with nursing, therapy, social services, physicians and the interdisciplinary team.
• Review clinical documentation to ensure accuracy of assessments.
• Support care plan development and interdisciplinary meetings for positive resident outcomes.
• Monitor Medicare and managed care reimbursement requirements and submissions.
• Assist with discharge planning to promote safe transitions and reduce readmissions.
• Provide education to staff on documentation and assessment requirements.
• Support quality improvement initiatives and regulatory readiness.
