Qualifications

• Current RN license.

• Knowledge of MDS 3.0, RAI guidelines, and care planning.
• Long-term care experience preferred.

Responsibilities

• Coordinate and complete MDS assessments accurately and on time.
• Maintain compliance with CMS, state, and federal requirements.
• Coordinate care plans with the interdisciplinary team.
• Work closely with nursing leadership and other departments to support quality resident care.
