Job summary

StoneGate Senior Living in Oklahoma City, OK seeks a MDS Resource Traveler (RN) or (LVN) to join our team. You will coordinate MDS and Care Plans, manage Medicare/Medicaid documentation, and ensure compliance with federal and state regulations.

The ideal candidate has an active RN or LVN license and 3–5 years in a skilled nursing facility, with experience in MDS processes and PPS. You will work within a patient‑focused, service‑driven environment that values quality care.

Qualifications

• Current RN or LVN license in the state of practice.
• 3–5 years of experience in skilled nursing facility with MDS/Care Plan work.
• Strong knowledge of MDS processes (RAI/PPS) and accurate documentation.
• Excellent attention to detail, organization, and communication.
• Proficiency with clerical tasks and computer systems.

Responsibilities

• Coordinate MDS/Care Plan schedules for the interdisciplinary team.
• Prepare MDS and Care Plans per regulatory requirements.
• Track Medicare resident benefit days and coordinate billing data.
• Respond to information requests from intermediaries and appeals.
• Prepare Medicare denial letters for residents discharged from Medicare coverage within time limits.
• Participate in Medicaid reimbursement documentation.
